Online Registration

NOTE: The Symposium has been postponed until May 23 - 27, 2022. Information on registration for the 2021 virtual activities will be posted as it becomes available.

Online registration for the 55th USPHS Scientific and Training is Symposium will open in early 2022 and will remain open through Noon on Thursday, May 26, 2022. Walk-in Registration will not be available onsite, only through the online registration system. Continuing education credits are included with registration.

Please Note: Registration for the retirement seminar is ONLY available with conference registration.

2022 Badges

PRINTED BADGES WILL NOT BE AVAILABLE ONSITE.
If you wish to wear a badge you may print it before arriving.  Badge holders and ribbons will be available for your use, or if you have a holder from a prior year you may use that.  Your badge and tickets may also be displayed on your mobile device for entry to sessions.

​To qualify, you must be a dues paying member of the Commissioned Officers Association of the USPHS. If you register at a discounted rate for which you are not eligible, then your registration rate will be changed to the Non-Member rate. No refunds will be granted.

Registration Cancellation Policy

​Refund requests will ONLY be considered in the case of medical emergency or deployment. You may email your request to symposium@phscof.org (Subject: 2022 USPHS Symposium Refund Request). Your request is not valid until receipt of it is acknowledged by the event planners in writing. All charges include a $35 non-refundable processing fee.
​Please note: We are no longer able to accommodate credit cards switches or refunding one card in order to charge another. Do not register until the payee and form of payment are final. No exceptions.  This means that if you think your agency might pay for you do NOT register with your own card.  You will not receive a refund from the Commissioned Officers Foundation.

Paying with Multiple Cards

If your organization is paying your registration fees, but you wish to add other items to your registration please follow the instructions below.
Registrant
1. Registrant should fill out the online registration form, selecting only what the organization will pay for, until they get to the payment page of registration.  The registration will be temporarily saved in the system.
2. Have the person paying for your registration complete the steps below.
3. Once notified by your Financial Officer that step 2 is complete, login to modify your registration to add and pay for the additional items.
Person Paying for Registration
1. Use the regular registration link, entering the name, email and registration type previously entered by registrant.  
1 a. If you would like to receive your own receipt, check the box "I am registering on behalf of this person," and enter your name and email address to be added as an administrator on the account.
2. Click next, then select "Yes, continue registering using the saved information," and click next again.
3. Click through all the registration pages until reaching the payment page.
4. Enter payment information and click Finish.

Modify a Registration

1. Click the Modify Registration button to the right.
2. Enter your email address and confirmation number then click Ok.
3. Click the Modify button under the title of the symposium.
4. Click through the registration pages making the necessary changes/additions.
5. On the payment page, enter your payment and click Finish.

Anchor & Caduceus Dinner

The dinner is NOT included in any registration rate. Tickets for the formal dinner cost $65 each and may be purchased through the online registration system.  Tickets will be available until the dinner sells out.  You will receive an email confirmation of your purchase once it is complete.  Paper tickets will not be given out onsite. You may either print your ticket(s) or show an electronic version of it on your mobile device at the door.  Tickets are non-refundable. Should you no longer be able to use your ticket you may transfer it to another person or donate it to someone on the waitlist.  To do so please contact us at symposium@phscof.org.
Attire
​"Active duty dress for the occasion is the Dinner Dress Blue Jacket or Dinner Dress White Jacket, without gloves. Officers with the rank of Lieutenant Commander and below may wear Dinner Dress Blue or Dinner Dress White (Service Dress Blue/White with black bow tie and miniature medals, no name tag). Civilian dress is formal - black tie tuxedo or dark business suit for men, and evening gown without gloves for women." (PHS Officer's Guide, 2nd edition, page 74)

Surgeon General's 5k Run/Walk

2022 information will be posted to the Run/Walk page.  The page is not currently available for the public to view, but will be reposted when current information becomes available.

Free Registration for Junior Officers and Civil Servants

Eligibility
  • PHS officer at grade O-3 and below or civil servant at equivalent GS 10/11 and below
  • Dues paying member of the Commissioned Officers Association of the USPHS​
  • Submit your Application/Register by the Deadline TBD for 2022.
Submission of Application
To submit an application, please use the regular registration link, and be sure to select Scholarship Application: Jr. Officer/Civil Servant as your Registration Type on the first page of the form.  After submitting the form/registration, if you meet the eligibility requirements you should assume you have received the scholarship.  We will only contact you if there is a problem/you do not qualify.
Without free registration, I would not be able to attend. I am in a remote location and travel alone costs a lot. I will return for the USPHS Symposium each year. Thank you."
